Submission Instructions
Adobe Reader 7 or greater is required to fill out this form.
To download the latest version of Adobe Reader, go to the Adobe Website: https://get.adobe.com/reader/
The first thing you will want to do is save the application file to an appropriate location on your PC or open it with Adobe Reader. Fill out the application by typing information in the appropriate fields. Required fields will be outlined in red. You will not be allowed to use the submit button until the required fields are filled. Once that is done, go to the top of the first page and click the Submit by E-mail button. A Select E-mail Client window will come up.
The Desktop Email radio button requires an email client such as Outlook, Outlook Express, or Thunderbird. This will open up a new email with the file attached and the To: line filled out.
If you do not have an email client set up on your computer and are using internet based email like Hotmail or Yahoo, choose the Internet Email radio button to save the file to a location on your computer – e.g. your desktop. You can then log into your email and attach the file like you would a photo.
If you have difficulty with the email button, you can now save the form (as it is filled in) to you computer and attach it to your email manually. To do this click the File menu, highlight Save As and select PDF. Save it to a location where you can easily locate it, such as your desktop.
